COVID-19: CREWS/PASSENGERS REQUIREMENTS AND RESTRICTIONS I. THE COUNCIL OF MINISTERS OF THE REPUBLIC OF POLAND HAS ISSUED A REGULATION ON THE ESTABLISHMENT OF SPECIFIC RESTRICTIONS, ORDERS AND PROHIBITIONS IN CONNECTION WITH THE STATE OF EPIDEMIC. II. TILL FEBRUARY 28 2022, AT 2359 LMT (FEBRUARY 28 2022, AT 2259 UTC) ACTIVITIES RELATED TO PASSENGER AIR TRANSPORT ARE ACCEPTABLE UNDER THE FOLLOWING CONDITIONS: 1) HAND DISINFECTION LIQUIDS ARE PROVIDED ON BOARD. 2) ACFT IS DISINFECTED: A) AT LEAST ONCE EVERY 7 DAYS, UNLESS THE ACFT HAS NOT BEEN USED FOR FLIGHT OPERATIONS WITH PASSENGERS SINCE THE PREVIOUS DISINFECTION, B) AFTER EACH FLIGHT WITH A SICK PERSON OR AFTER EACH FLIGHT FROM THE TERITORRIES OF HIGHT SARS-COV-2 INFECTION RATES ACCORDING TO THE AIR OPERATOR'S RISK ANALYSIS. 3) PASSENGERS ARE INFORMED THAT BEFORE THE CHECK-IN THEY HAVE TO FILL OUT THE ELECTRONIC PASSENGER LOCATOR FORMS AVAILABLE ON THE WEBSITE HTTPS://WWW.GOV.PL/KLP OR IF THIS IS NOT POSSIBLE, THE AIR CREW DELIVERS TO PASSENGERS THE PASSENGER LOCATOR FORMS IN PAPER AND COLLECTS THE FILLED FORMS - IN CASE WHEN THE PASSENGER CROSSES THE STATE BORDER, TO GO TO THEIR PLACE OF RESIDENCE OR STAY IN THE TERRITORY OF THE REPUBLIC OF POLAND. 4) PASSENGERS ARE VERIFIED THAT THEY HAVE FULFILLED THE OBLIGATION TO SUBMIT THE ELECTRONIC PASSENGER LOCATOR FORM. COVID-19: TO ENSURE SAFE AND EXPEDITIOUS DELIVERY OF COVID-19 VACCINES, AIRCRAFT OPERATORS OF FLIGHTS CARRYING SUCH VACCINES SHOULD ASK APPROVAL FROM CAA OF POLAND (CAAPL(AT)ULC.GOV.PL) FOR EXEMPTION FROM ATFM MEASURES FOR EACH FLIGHT DEEMED CRITICAL. UPON APPROVAL, STS/ATFMX AND RMK/VACCINE SHALL BE INSERTED IN FIELD 18 OF THE FLIGHT PLAN. 1. Until 10 January 2022, flights from Botswana, Eswatini, Lesotho, Mozambique, Namibia, South Africa and Zimbabwe are suspended.

2. Passengers are not allowed to enter. - This does not apply to: - nationals and residents of Poland; - spouses and children of nationals of Poland; - passengers under the

constant care of nationals of Poland; - nationals of Austria, Belarus, Belgium, Bulgaria, Croatia, Cyprus, Czechia, Denmark, Estonia, Finland, France, Germany, Greece, Hungary,

Iceland, Ireland (Rep.), Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Malta, Netherlands, Norway, Poland, Portugal, Romania, Slovakia, Slovenia, Spain, Sweden, Switzerland

and Ukraine; - British nationals; - spouses and children of nationals of Austria, Belgium, Bulgaria, Croatia, Cyprus, Czechia, Denmark, Estonia, Finland, France, Germany, Greece,

Hungary, Iceland, Ireland (Rep.), Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Malta, Netherlands, Norway, Poland, Portugal, Romania, Slovakia, Slovenia, Spain, Sweden and

Switzerland; - spouses and children of British nationals; - passengers with a diplomatic passport; - nationals and residents of Australia, Canada, Georgia, Israel, Japan, Korea

(Rep.), New Zealand, Thailand, Tunisia, Turkey and USA; - passengers with a permanent residence permit or a long-term residence permit issued by Austria, Belgium, Bulgaria,

Croatia, Cyprus, Czechia, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Iceland, Ireland (Rep.), Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Malta,

Netherlands, Norway, Poland, Portugal, Romania, Slovakia, Slovenia, Spain, Sweden, Switzerland or United Kingdom and their spouses and children, returning via Poland to their

country of residence; - passengers arriving from Austria, Belgium, Bulgaria, Croatia, Cyprus, Czechia, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Iceland, Ireland

(Rep.), Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Malta, Netherlands, Norway, Portugal, Romania, Slovakia, Slovenia, Spain, Sweden or Switzerland; - merchant seamen; -

passengers traveling as students; - passengers with a Polish Card (Karta Polaka).

3. Passengers entering or transiting through Poland without a negative COVID-19 test result in English or Polish issued at most 24 hours before arrival are: - subject to a

COVID-19 test upon arrival; or - subject to quarantine for 14 days. - This does not apply to: - passengers arriving from Austria, Belgium, Bulgaria, Croatia, Cyprus, Czechia,

Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Iceland, Ireland (Rep.), Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Malta, Netherlands, Norway, Poland,

Portugal, Romania, Slovakia, Slovenia, Spain, Sweden or Switzerland; - Kindergarten, preschool and primary school pupils.

4. Passengers must complete a "Passenger Locator Form" (PLF) and present it upon arrival. The form can be found at https://aplikacje.gov.pl/app/klp/#/home or on board.

5. Passengers could be subject to quarantine for 10 days. - This does not apply to passengers with a COVID-19 vaccination certificate in English or Polish showing that they were

fully vaccinated at least 14 days before arrival. Vaccines accepted are: AstraZeneca (Vaxzevria), Janssen, Moderna and Pfizer-BioNTech. More quarantine exemptions can be found at
