4. Passengers must complete a "Public Health Travel Declaration Form" and "Passenger Locator Form" found at https://app.euplf.eu/#/ and present it upon arrival. - This does not

5. Merchant seamen must have a negative COVID-19 RT-PCR test taken at most 72 hours before departure. The test must be based on an oropharyngeal swab and the test result must be

in English or accompanied by a certified English translation.

6. Passengers could be subject to quarantine in a hotel for 14 days at their own expense. - This does not apply to passengers with: - a negative COVID-19 PCR test taken at most 72

hours before arrival. The test must be based on an oropharyngeal swab; or - a COVID-19 recovery certificate issued at most 6 months before arrival and a COVID-19 vaccination
