The Czech Ministry of Health launched an online form mandatory for travellers arriving in the Czech Republic from all countries not featuring on the 'safe countries list' (the link to the portal is https://plf.uzis.cz/). Filling in the online form is mandatory to all travellers from at risk countries; failure to provide the filled in form can result in the passenger being prevented to onboard the flight.
